(1) The application for admission as member of the Fund shall be in Form No. I. The duly filled and signed application shall be submitted before the District Welfare Officer of Lottery with a registration fee of Rs.25. (2) Every application for admission as member by a seller shall be recommended by a registered Trade Union in the field of lottery. (3) In case of a seller, Certificate of Authorisation from the concerned agent in Form No. II or the Sellers' Ticket Account Book duly authenticated by the concerned agent as proof of purchase of minimum number of tickets shall be accompanied with the application. (4) The agent/seller intending to obtain membership shall furnish along with the application, documents to prove age, date of birth and address. The attested copy of the concerned page of the School Admission Register/Secondary School Leaving Certificate/Voter Identity Card/PAN/Ration Card/Passport/a Certificate from the Medical Officer not below the rank of an Assiatant Surgeon shall be considered as documents to prove this. (5) Each member shall be issued an Identity Card in Form No. III by the Registering Authority on registration as a member in the Fund. (6) Production of Certificate from District Lottery Officer in proof of the purchase of minimum number of tickets along with the application for membership by an agent.
