Each member shall give name, address and relationship of the nominee to whom the benefits shall be payable in the event of his death in part B of the application form: Provided that the member can nominate person or persons from among his family members as defined in sub-clause (g) of clause 2 of the Scheme: Provided further that the member can change his nomination only twice in the period of membership in the scheme.
