It shall be the duty of the every staff member, subordinate to or under the orders of the Superintendent, to make an immediate report to that officer of any misconduct, or of willful disobedience or breach of the provisions of any law, rules or regulations for the time being in force, committed by any other staff member or any prisoner which may at any time come to his knowledge or be committed in his presence, sight or hearing. No officer to enter any ward or cell alone, from lock-up to sunrise.
