(1) The history-ticket of every prisoner shall contain the following entries, namely:- (a) date of admission to prison; (b) number and name of every article of clothing and equipment, issued; (c) work and task to which the prisoner is put; (d) any change of work or task other than medical grounds; (e) action taken on any direction or recommendation of the medical officer; (f) application for a copy of judgment, if the prisoner desires to appeal; (g) receipt of the copy of judgment; (h) dispatch of appeal or revision or petition; (i) substance of the order of the appellate court; (j) fact of an appeal not having been made before the expiration of the term allowed for appealing; (k) amount of remission awarded quarterly, in case of special remission, number and date of orders of the competent authority to be mentioned; (l) total remission in days earned up to the end of each quarter; (m) prison offence if any, committed; (n) exceptional interview if any, allowed; (o) dispatch to a Court, or transfer, discharge, escape or death; (p) any recommendation of the Deputy Superintendent or the Assistant Superintendent; (q) list of visitors expected; (r) action taken on any order passed by the Superintendent; (s) location in a cell by day or night; (t) particulars of confinement in high security cells and reasons thereof; (u) details of every release from and surrender to the prison. 1220 HARYANA GOVT. GAZ. (EXTRA.), DEC. 30, 2022 (PAUS. 9, 1944 SAKA) (2) The entries (a), (b), (e), (f), (g), (h), (i), (j), (l), (m), (n), (o), (p), (q), (s), may be made by the Assistant Superintendent. Entry (k) may be made by the Assistant Superintendent, or any other officer authorized to award remission. Entry (c) and (r) shall be made by the Deputy Superintendent (Administration). The duty of making the entries (d) and (t) shall not be delegated to any officer subordinate to the Deputy Superintendent. NOTE 1.- No adverse entries shall be made on the history-ticket without the orders of the Superintendent. NOTE 2.- Every entry in the history-ticket shall be attested and signed by the officer concerned with his name and seal. Entries to be recorded by Superintendent.
