(1) The medical officer shall, in the history-ticket of every prisoner, enter or cause to be entered under his supervision the following, namely:- (a) the prisoner’s weight on admission; (b) brief medical history and present health condition; (c) the class of labour for which he is fit, if sentenced to labour, along with the reasons therefor; (d) brief description of his present mental state. (2) The medical officer shall also subsequently enter or cause to be entered the following, namely:- (a) any sickness or ailment reported and the line of treatment alongwith details; (b) date of admission to and discharge from hospital on every occasion, with particulars of the disease (s) for which admitted alongwith treatment; (c) particulars of admission to and discharge from the convalescent group; (d) necessity for segregation of the prisoner, if any; (e) vaccination details of the prisoner, if any; and (f) half yearly weighments. (3) the medical officer shall himself enter such other directions or recommendations as he may from time to time consider necessary for the maintenance of the health of the prisoner.
