(1) A history-ticket for each prisoner shall be prepared immediately on his admission into prison and it shall be maintained in the manner hereinafter provided, throughout the period during which such prisoner remains in confinement. (2) The history–ticket may also be maintained digitally. (3) Every history-ticket shall contain the following details amongst others, - (a) the unique prisoner number, name, residence address, and other particulars necessary for the identification of the prisoner along with his photograph; (b) security category of the prisoner; (c) In case of a convict, - (i) the nature of the offence for which he has been convicted and the provisions of the law applicable thereto; (ii) FIR Number, date, police station, and brief facts of the case; (iii) the date, sections, nature and extent of the sentence passed; (iv) period undergone under section 428 of the Code of Criminal Procedure, 1973(Central Act 2 of 1974); (v) detailed previous history of convicted including decided and pending cases along with the gist of each case; (vi) name and details of the prisoner’s nominee who is to be handed such prisoner’s property, wherever applicable; (vii) prisoner’s accommodation details in the prison; (viii) details and status of appeal or revisions filed by the prisoner, if any; (ix) particulars of his lawyer in each case, appeal or revision; (x) educational qualifications and skills (at the time of admission to prison and acquired while in prison); (xi) details of duty as convict officer. (d) The history-ticket of every under-trial prisoner shall contain the following particulars, namely: - (i) FIR number alongwith date and section(s); (ii) name of the police station where the FIR is registered; (iii) brief facts of the case; (iv) name of the trial court; (v) date of hearing; (vi) details of all criminal cases decided and pending, including appeals and revisions, name and particulars of the legal counsel etc. (4) Every entry made on the history-ticket shall be so made at the time of, or as soon as possible after, the occurrence of the event to which it relates, and shall be signed with name and date by the officer who maintains it. (5) A duplicate history-ticket shall be issued in case the original history-ticket is lost. The new history-ticket shall be marked duplicate and duly signed/certified by the competent authority. The ticket shall be reconstructed from available records maintained under these rules or otherwise. HARYANA GOVT. GAZ.
