In Lhc ease of a person who has been attended in his last illness by a duly qualified medical practitioner, such praciiiioner shall, wiihin lliree days of his becoming cognisanl of lhe dealh or such person, sign and forward lo the -'[orficer-in-charge as specified in sub-seclion (1) of scclion 451] a ccrtificaLc of the cause of such person's dealh in such form as shall from lime Lo Lime be specified by the Municipal Commissioner in this behalf, and Ihe cause or dealh as staled in such cerlifiealc shall be enlered in the regisier togelher wiih Ihe name or Lhe certifying medical praciiiioner.
