The application under rule 146 shall contain. Application for deposit (a) a statement of the ground on which it is made; (b) the name of the person or persons to whose credit the deposit is to be entered; (c) the name of the person or persons to whom the rent is due; and (d) the name of the person or persons to whom rent was last paid and of the person or persons now claiming it.
