Application forms can be obtained from either of two offices i.e. State Govt./U.T. concerned or Freedom Fighters' Division, Ministry of Home Affairs, 1st Floor, Lok Nayak Bhavan, New Delhi free of cost. All State Govt./U.T. Administrations have been instructed to make available the prescribed application forms in sufficient number at all levels including Block/Tehsil level.
