(a) Filing of application and disposal- [Workers desirous of joining the Scheme will submit application before the District Executive Officer, Kerala Motor Transport Workers Welfare Fund Board in whose jurisdiction the applicant works/employed, in duplicate, in the prescribed form (vide Appendix I) along with two passport size latest photographs of the applicant and a certificate obtained from an office bearer of a registered Trade Union functioning among the Autorickshaw Workers/ Mps/Member of Local Bodies or any officer in the Kerala State Labour Department not below the rank of Assistant Labour Officer Grade II, to the effect that the applicant is a worker as defined under clause 2 (1)(a) of the Scheme. The District Executive Officer, will scrutinise the application and verify, the correctness of the information and pass appropriate orders in each case. He will communicate the order to the applicant and to the Autorickshaw owner concerned. The administrative Committee under clause 3 of the Scheme will have the powers to cancel the membership of any person, if any fraud is detected.] (b) The District Executive Officer will send a copy of the order along with one copy of the application received from the applicant with his remarks leading for the order under sub. clause (a), to the Chief Executive Officer. (c) Procedure for remittance of contribution- On receipt of the intimation/ order that the applicant has been accepted the worker will remit rupees [twenty five] as his contribution in the nearest branch of the Bank as directed in the order of the District Executive Officer, every month, Before money is accepted the Bank will ensure that proper order of the concerned officer is produced. (d) The date on which he remits money/contribution will be the date on which he is deemed to have been enrolled as a member of the scheme. (e) A pass book showing credit of the contributions of each member in the bank shall be arranged to be provided to the members by the bank.
