Information as to change of Address of Clinical Establishment :— Any change in the address or situation of, or of staff belonging to, a clinical establishment shall be communicated by the owner, proprietor or management, thereof to the supervising authority 1["not later than fifteen days"] after such change with specific mention as to the exact date when such change occurred.
