(0 That the applicant has a feasible and time bound programme to set- up the Sansihnn along with required infrastructural facilities including adequate hostel facilities lor boys and girls as prescribed by the council which commensurate with the proposed intake of students so as to complete the Sansthan within a period of four years for degree, two years for diploma and one year for certificate courses from the date of grant of such permission. (g) (i) The Sah Chikitsiya Shikshan Sansthan shall admit students only sifter the Pnrishad is satisfied about infrastructural and other facilities for starting such courses. {iik If in future the recognition of any Sansthan is withdrawn on any ground then the Pnrishad shall have the right to transfer the students of such Sansthan to any other recognized Sansthan, (h) That the applicant has the necessary managerial and financial capabilities to establish (he proposed Shikhan Sansthan and its ancillary facilities like teaching staff etc. (i) That the'applicant institution along with application shall provide three years audit report of the chartered Accountant regarding its financial position, (j) That the applicant shall submit a bank guarantee of rupees three lacs for one to three courses. I'i ve lacs for one to ten courses and for more than ten courses in addition to five lacs, at the rate of two lacs for each additional course for all subjects in favour of the Parislutd, for fulfillment infrastructural, teaching and other norms fixed hy the Parishad.lf the Parislutd is of the opinion that the institution is not working as pet titles, then the bank guarantee submitted by the institution shall be forfeited to by the Pnrishad. i Exception.—The above condition shall not apply to applicants who are .State Government provided they shall give an undertaking to provide funds in their budget regularly and give grant-in-aid till facilities are fully provided as per the time bound programme indicated by them. 5, Forms and Procedures.—Subject to the fulfillment of the eligibility criteria and qualifying criteria, the application for permission to establish a new Para-Medical Educational Institution shall he submitted by the applicant m the following three parts :■— PART—A Part - A—The application (Appendix) shall contain the following particulars oft lie applicant and information regarding desirability and prim a facie feasibility of setting tip an educational institution at the proposed location :— (if Information regarding consl-itution of applicant's organization within the terms of reference of eligibility criteria', (ii) Information regarding1 basic infrastructural facilities, managerial and financial capabilities of the' applicant; and (iii) Information regarding, availability of necessary certificates and consents as prescribed in the qualifying criteria. wra, firm; 26 2009 -'144(9) (c) Details about the udditional land for expansion or tcif setting-iip new hospital, including land particulars, distance from the proposed educational institution, plot size, authorized land usage, soil condition, road access availability of public transport,, electric supply, water supply, sewage connection, telephone lines etc. UD Up-graded medical programme which includes additional clinical and Para-clinical disciplines envisaged under the expansion scheme. (c) Up-graded functional programme which includes specialty-wise and service wise functional requirements and area distribution and specialty wise bed distribution. (0 Building programme which includes details of additional built-up area of hospital, staff housing, hostel of students and other ancillary buildings. (g) Planning and layout which includes upgraded master plan of the hospital complex or master plan of the proposed new Inapital as the case may be, along with layout plans, sections, elevations and floor wise area calculation of the hospital and ancillary buildings. (h) Details of upgradation or addition in the capacity, configuration of services and hospital services. (i) Upgraded Equipment pro gram me which includes up-graded room-wise list of medical and allied equipment, schedule of quantities and specifications. (j) Upgraded manpower programme which includes category wise distribution of medical, paramedical and other staff. (k) Expansion scheme which includes month wise Master Schedule of activities indicating commencement and completion of building, local body approval, civil construction, provision of medical and allied equipment, recruitment of staff commensurate with the development of proposed educational institution. (l) ^Project cost of expansion scheme which includes additional cost of land, building, hospital services, medical ? and allied equipment, furniture and fixture and preliminary and pre-operative expenses. (rn) 5.Means of financing the cost of expansion scheme which includes contribution of applicant and grams, donations, ioans received and other sources, if any. (r>) - Revenue assumption which includes details of income from various procedures and services, upgraded • service loads and annual revenue. (o) Expenditure assumptions which includes operating expenses and depreciation, and (p) Operating result which includes Income Statements. Cash flow statement..and balance sheet.
