(1) When a dock worker contracts any disease as specified in Schedule IV a notice in prescribed form shall be sent by the employer and Port Authority forthwith to the Chief Inspector. (2) If any medical practitioner attend on a dock worker suffering from any disease specified in Schedule IV the Medical Practitioner shall, without delay, send a notice to the Chief Inspector stating the name and full particulars of the patient and the disease in the specified pro forma in Schedule XII.
