The Case Management Framework is required to ensure continuity in a prisoner’s rehabilitative process before and after the prisoner is released. The service involves, - (a) assessing a prisoner’s individual needs; (b) identifying appropriate services that meet his needs; (c) developing a comprehensive plan of service tailored to the individual; (d) advocating the client’s access to and use of services; (e) monitoring and evaluating the effectiveness of delivery of services. Treatment of after care problems.
